Procedure Description:
Rhinoplasty techniques have changed greatly over the past thirty years. Older techniques generally involved reducing the cartilage framework, thereby making it weaker and less able to withstand the forces of wound contracture. This woman’s nose was an example of just that. Her prior rhinoplasty led to pinching at her tip, collapse of her side walls, over-rotation and deformation of her tip. As a result, she was unable to breathe without the use of external tape. Modern techniques involve maintaining structure or augmenting it to create more reliable, natural-looking outcomes. Correction of this woman’s nose involved the use of cartilage harvested from her chest wall. Her tip cartilages had been divided at the domes and were reconstituted. Her tiny lateral crural remnants were replaced and repositioned and her middle vault was reconstructed. Her nasal bones did not require any manipulation. In addition to correcting her nose, I performed a lower eyelid surgery. She is thrilled with her beautiful outcome, seen here 18 months after surgery, and even more so that she has regained the ability to breathe easily from her nose.
